Football - Concussion Contract (Mandatory)
Parents,along with the athlete, must read and sign the Concussion Contract. They must sign that they have read it and understand it. Once completed, it must be turned into the coach AD or send directly to the CYO office. All forms that are collected are to be turned into the CYO office. This is a law in the state of Washington. If one of our teams makes it the semi finals and/or finals of the playoffs which are played at Kiggens bowl in Vancouver, Washington and a player and his parents do not have this form signed they will not be able to play in the game. Concussions are being taken very seriously due to the brain damage they cause. If a coach suspects a concussion they are to immediately take the kid out of the game or practice.
